What’s Changing:
Automatic Self-Certification: With our new system, your DOT Medical Certificate will be automatically submitted and self-certified with the MA RMV — eliminating the need for you to handle that step separately. This update streamlines the entire process, making it more convenient and efficient for our patients.
New Pricing: Due to the new FMCSA program and automated certification service, there will be a $10 price increase for all DOT Physicals starting May 19, 2025.
CDL Drivers: Important Self-Certification Notice
Are you receiving notices from the Registry that you are NOT self-certified and that your CDL is being downgraded?
On 12/15/2025, the FMCSA released information confirming that not all self-certifications are being transmitted to State Driver Licensing Agencies (SDLA).
At our office, all self-certifications are processed electronically through Medfiller. However, over the past several months, we’ve noticed that some certifications appear in the National Registry but do not show in the State Registry. We have reported this issue multiple times to both the FMCSA and the Registry.
Good news: As of December, the FMCSA has implemented a workaround for drivers who completed their physical but are not appearing in the State Registry.
What drivers will need:
• A screenshot from the National Registry showing your information is uploaded to FMCSA
• A copy of your DOT Medical Card
• The SDLA instructions (link provided below)

What You Need to Do Before Your Appointment:
To ensure a faster check-in process, we kindly ask all patients to:
Contact the office in order to schedule an appointment. You will receive an email with your instructions from www.medfiller.com
Log in and complete your patient profile, including your address and medical history, before your visit.
This step is required in advance and significantly reduces wait times at check-in.
For Walk-In Visits:
If you’re coming in as a walk-in, you will be provided with a QR code upon arrival. Scanning this code will allow you to complete the information necessary for your appointment.
