The Medical Physical and Paperwork is $350.00 for the physical only. Lab work is an additional charge (see below)
Lab work depends on the age and on the vaccinations and immunizations records that you provide me with. Health insurance can be used for this portion of the exam.
We no longer provide vaccinations/immunizations, you will need to obtain them from your primary care physician or the pharmacy. All local pharmacy’s, Walmart and Target will have the vaccinations. For those paying out of pocket, we advise to get a Good RX card (www.goodrx.com) and search for the cheapest price in your area. We will provide a list of the local pharmacies with the cheapest prices.

Gonorrhea $28.00 (ages 18 to 24)
RPR $5.00 (ages 18 - 44)
Hepatitis B $15.00 (Birth - 59 years of age)
MMR $35.00 (all ages born after 1957)
Varicella $15.00 (all ages)
